Transform Homecare is a service provided by Transform Housing & Support. We have operated a home care service since 2005 and currently provide care for more than 190 people across Surrey and parts of West Sussex. We give care to help people live as independently as possible in their home, either in the community or within extra-care settings. 

Position: Supervisor - Care & Support, Extra Care 
Location: Sunbury TW16 
Job type: Full Time, Permanent 
Hours: 37.5 hours per week 
Salary: £23,558.00 per annum + overtime payments + benefits 
Benefits: Defined Contribution Pension Scheme. Travel expenses at 45p per mile. 25 days annual leave plus Bank Holidays. Transform Housing & Support will pay for an enhanced DBS, Uniform, Training and the Employee Assistance Programme. 

About the role: 

The Supervisor - Care & Support, Extra Care will manage a team of staff to deliver a professional, caring and CQC compliant service to older and vulnerable clients within an Extra Care Scheme located in Sunbury-on-Thames. This will include some personal delivery of personal care at times and a willingness to work outside of normal office hours as the needs of the service require. 

Ensuring clients’ privacy, dignity and confidentiality at all times our Supervisors liaise on a regular basis with clients, their relatives or advocates, and a range of other professionals. Our Supervisors are also responsible for devising and reviewing support plans for clients which offer the best options for enhancing their independence as well as ensuring their safety and security. 

About you: 

You will have a good general education to GCSE standard or equivalent, with an NVQ3 or higher being desirable. You will have experience of working in the care and supported living environment as well as knowledge and understanding of support planning and risk assessment. 

Experience of managing a team and liaising with external stakeholders is desirable. 

As well as good written and oral communication skills, you will have the ability to plan and prioritise workloads. It is also essential that you have IT skills including MS Office Word and Outlook. 

You must have a full valid driving licence, as well as use of your own car for work purposes. 

Enhanced DBS clearance will be required.
