Position: Dementia Support Worker

Hours: 21 per week
Salary: £12,207 actual per annum

About the role:
We are looking for an exceptional individual to join our Dementia Support team. You will provide support and guidance to people with dementia and their carers across Gwent, helping to maintain their independence, improving their sense of well-being, and putting them more in control of their lives. The service is unique to each service user as it is based upon their personal circumstances and support needs.
Within this role you will be expected to facilitate a number of Alzheimer’s Society support and information programmes, this includes the “Live well with dementia programme (LWwd)” and the “Carer Information and Support Programme (CrISP)”. You will help provide families and carers with the information and support that enables them to care better for the person with dementia and links them to local services.
Another part of this role includes facilitating the “Focus on Dementia Network Meetings”, these groups actively engage people affected by dementia in influencing all aspects of the Society’s work and the wider health and social care environment.
We are looking for an enthusiastic individual who possess NVQ level 3 (or equivalent) with an understanding of dementia and the needs of those affected by dementia. An ability to assess the service user needs along with an understanding of the importance of confidentiality is essential.
To be successful in this role, you will have excellent communication skills. This role requires regular use of IT packages, therefore a good standard of IT would be desirable.
You will be responsible for your own caseload therefore the ability to manage your own time effectively is essential. Your caseload will be varied so we are looking for someone with the right mix of enthusiasm, professionalism and flexibility who is able and willing to work in a variety of settings.
