Alzheimer's Society is the UK's leading dementia charity. We provide information and support, improve care, fund research, and create lasting change for people affected by dementia.
We are looking for an empathetic and knowledgeable individual to join our new team of Dementia Navigators in Southwark. You will be working across the local care networks and working with GP’s, Social Workers and the memory service to provide information and advice to people affected by Dementia and their carers. Previous experience of working with people affected by Dementia is desirable. You will need to have experience of working within health and social care and experience of carrying a busy caseload. Basic IT skills are essential.
You will be working with a variety of professionals across health and social care to provide those affected by dementia the best service possible.
You must also be an excellent communicator and listener. Excellent organisational skills are essential, and you are likely to have experience of working with statutory and voluntary agencies. You must be able travel independently and work flexible hours when necessary, this could include evenings and weekends and you will quickly establish yourself as a vital member of our team.
