About the role: 

We are looking for experienced and highly professional individuals to join our Dementia Information and Support Service in Surrey. The service is specific to each person's needs and is decided in agreement with them. 

As a Dementia navigator you will provide one to one support to people with dementia and carers and this can be provided face to face, over the telephone or via written communication. Dementia Navigators provide information, guidance and practical support to help people understand dementia, cope with day to day living with dementia and prepare for the future. The service also supports people to access other services. 

About you: 

We are looking for individuals with excellent communication, organisational and people skills. This is a great opportunity for those that really want to make a positive difference to the lives of those affected by dementia. 

You will possess NVQ level 3 or demonstrable experience and have competent IT skills. It is desirable that you have experience of delivering services in a health or social care setting and that you have experience of working with statutory and voluntary agencies.
