Position: Information Advice and Support Coordinator
Location: Slough, Berkshire
Contract type: Fixed Term to 30 September 2017 with possibility of extension
Hours: 35 hours per week
Salary: £22,397.16 to £24,524.97 per annum

About the role:
We are seeking an innovative, passionate and professional individual with excellent interpersonal and organisational skills to join our team in East Berkshire.
Reporting to the Area Manager for Berkshire East, the Service Coordinator will join an experienced team in supporting new stroke survivors and their carers from hospital discharge into the community.
The coordinator will provide regular reviews and create client-centred intervention plans to support service users to achieve their desired outcomes. Key aspects of the role include providing tailored information and advice and recruiting and managing a team of volunteers.
You will thrive in a busy environment with a proven track record of effective time management and have experience working within the health and social care sector. You will have excellent IT skills and a strong understanding of how to use data to improve quality. A flexible approach to your role and the ability to drive is essential.
