About the role
Our dedicated local teams provide a holistic pathway of post diagnostic support in Greater Preston, Chorley and South Ribble and West Lancashire for people with dementia and their families. To continue meeting the needs of people with or affected by dementia, we are continuously developing the range and quality of our services - and that calls for professionals, like you, who can help us achieve the very highest standards of efficiency, innovation and service.
We are looking for a experienced and extremely well organised practitioner to provide timely and effective support for people living with dementia and their families. The role is a demanding one with your services being required across the patch, it is essential you have an understanding of the needs of people living with dementia and their families.
